Post Acute Care and the Physicians’ Role

The Population Health Management (PHM) strategy is to identify high-quality post-acute care providers to actively participate in an integrated network of care with Covenant Health Partners (CHP) and Covenant ACO. Five Home Health agencies and four Skilled Nursing Facilities have executed PAC Affiliate Provider Care Agreements with Covenant.

Home Health Affiliate Providers:

1. Calvert Home Health
2. Beyond Faith Home Health
3. Caprock Home Health
4. Interim Home Health
5. CenterWell
6. Enhabit Home Health

Skilled Nursing Facility Affiliate Providers:

1. Crown Point
2. Carillon Health Suites
3. Lubbock Health Care

As a physician you are not required to use or refer to the PAC Affiliate providers; the selection of post-acute providers is ultimately up to the member. However, if a PAC Affiliate provider is selected to provide post-acute care, that care will be supported and enhanced. The affiliate providers are expected to achieve target metrics for the following:

1. CMS Star ratings from Nursing Home and Home Health Compare.
2. Total cost of care.
3. Average cost per case.
4. Emergency Room and Inpatient readmission rates.
5. Length of stay.
6. Case Mix Index.
